    Simultaneous versus staged resection for synchronous liver metastases from colon cancer

    Objective: To compare the clinical efficacy of simultaneous and staged resection for colon cancer with synchronous liver metastases. Methods: The clinical data of 36 patients with synchronous liver metastases from colon cancer were retrospectively analyzed. Of the patients, 16 cases underwent radical resection of colon cancer and simultaneous resection of liver metastases (simultaneous resection group), and the other 20 cases underwent radical resection of colon cancer and two-stage resection of liver metastases (staged resection group). The 1-, 3- and 5-year survival rates between the two groups were compared. Results: The 1-, 3-, and 5-year survival rate in simultaneous resection group was 87.5%, 37.5% and 18.8%, and in staged resection group was 65%, 10.0% and 0.0%, respectively. By comparison, the 1-year survival rates had no statistical difference between the two groups (P>0.05), but both 3- and 5-year survival rates in simultaneous resection group were significantly higher than those in staged resection group (both P<0.05). Conclusion: Simultaneous surgery can prolong the survival time of patients with synchronous liver metastases from colon cancer.

    Aptamer KMF2-1a-based chemotherapeutic drug carrier targeting breast cancer cells

    Objective: To investigate the feasibility of the development of aptamer KMF2-1a-based doxorubicin carrier that targets breast cancer cells. Methods: The internalization efficacies of aptamer KMF2-1a and its modified product (drug carrier) into human breast cancer MCF-10AT1 cells were tested by flow cytometry analysis. The incorporation of doxorubicin into the drug carrier was determined by spectrophotometer analysis after their interaction. Results: Flow cytometry analysis showed that both aptamer KMF2-1a and its modified product could be specifically internalized by MCF-10AT1 cells. Spectrophotometer detection demonstrated that doxorubicin was successfully incorporated into the drug carrier. Conclusion: Aptamer KMF2-1a can be internalized specifically by MCF-10AT1 cells, and it can be potentially used as a doxorubicin carrier after modification for targeted breast cancer therapy.

    With the development of day surgery promoted by the national health care reform, the proportion of day surgery in the elective surgeries is becoming increasingly higher. The ordinary medical record templates are difficult to meet the clinical requirements of the development of day surgery, due to its short length of hospital stay, multiple disease entities, rapid turnover of patients and heavy workload in writing the medical records. At present, there is no  uniform norm or standard for medical record writing of day surgery in our country. For promoting and standardizing the development of day surgery, the experts of the National Clinical Research Center for Geriatric Disorders (Xiangya) developed the Expert Consensus on the Norms for Writing Medical Records of Day Surgery, based on the Xiangya Norms for Writing Medical Records of Day Surgery (2018) and in accordance with the Basic Norms for Writing Medical Records (2010) proposed by the previous Ministry of Health of China and the Guidelines for Management of Day Surgery (2016 exposure draft) recommended by the National Health and Family Planning Commission.

    Abstract:Objective:To investigate the effects of oxamate on concentrations of calcium in pancreatic cancer cells.
    Methods :Pancreatic cancer cell panc-1 was cultured in vitro with oxamate for 48 h, then stained with Fluo-3/AM, and the light density of cells for different concentration of oxamate under confocal laser microscopy was observed.
    Results:Oxamate influenced the concentration of calcium in panc-1,and this effect corresponded to the concentration of oxamate.
    Conclusions:Oxamate can induce the increase of the concentration of calcium of panc-1,and then influence the cellular pathophysiologic process.

    Abstract:Objective:To establish a stable primary hepatocellular carcinoma(HCC) model of C57BL/6J mice, and establish a basis for indepth laboratory research in hepatocellular cancer.
    Methods :Ninety C57BL/6J mice were induced to establish HCC models by combination of diethylnitrosamines (DEN), carbon tetrachloride (CCl4) and ethanol for twenty weeks, then the occurrence and development of HCC were observed, and other 10 C57BL/6J mice were used as normal control(no drug was given). RTPCR method was used to observe AFP mRNA expression in liver tissues and tumor tissues.
    Results:seventyone of 90 mice developed HCC(78.9%), cirrhosis was observed in 11 of 90 mice(12.2%), and 8 of them died of toxic hepatitis and acute hepatocellular necrosis. AFP mRNA was expressed only in liver cancer tissues. No obvious liver pathologic changes were observed in the control group. The histopathologic changes of HCC were high or middle differentiation.
    Conclusions:An AFPsecreting C57BL/6J mouse liver cancer model was established by induction with combination of DEN, CC14 and ethanol in a relatively short time. The rate of cancerous change was high, and some of the hepatic changes included hepatitis and cirrhosis. This is a rather ideal animal model for the study of liver cancer.

    Improvement of surgical procedure for benign breast tumor

    Objective:To investigate the surgical technique of reducing scar and maintaining the mammary shape in resection of benign breast tumor. Methods:In these patients, a curved incision was made over the tumor or around the mammary areola if the tumor was close to the mammary areola; the residual breast cavities preserved without suturing; 4-0 absorbable sutures were used for subdermal continuous suturing, then a pressue package was made; and no drains were used. Results:A total of 542 patients with 613 benign breast tumors were resected. No incision infection and dehiscence occured after operation and only linear scars were left at the surgical sites. Hematoma occrred in 26 patients (4.24%) and it resolved after conservative treatment. Skin depression due to relatively larger resection area developed in 3 patients (0.49%). Conclusions:This operative procedure not only simplifies the process and reduces the operating time, but also ensures a natural outline of the breast.

    The progress of liver surgery is characterized by precision, minimal invasion and effectiveness in the early 21st century. Hepatectomy, as the mainstay treatment for hepatocellular carcinoma (HCC), has entered a precise era. However, key techniques involved in the precise hepatectomy for HCC remain unclear at present. The author, based on the systematic clinical investigations, proposes that the key techniques involve four aspects: accurate assessment and planning, precise dissection of the porta hepatis, meticulous hepatic parenchyma transaction and sophisticated postoperative management. There is an urgent need for the standardization of the key techniques of precise hepatectomy for HCC, and the evaluation of the efficacy and safety of precise hepatectomy for HCC from the perspective of evidence-based medicine, so as to further disseminate the precise hepatectomy as an appropriate technique in the country.

    Epithelial mesenchymal transition (EMT) is an important biological process enabling the malignant tumors of epithelial cell origin to acquire migratory and invasive ability. Among the mechanisms contributing to EMT, transforming growth factor β (TGF-β) is considered to play a key role. This paper overviews the relationship between EMT and tumor invasion and metastasis, and mentions the related signaling pathways mediated by TGF-β, which are responsible for EMT.
